The objective. A clinical-immunological rationale of using a vitamin-mineral complex and an immunomodulator Imunofan to optimize therapy of the convalescent period of haemorrhagic fever with renal syndrome. Patients and methods. We studied the efficacy of Imunofan, Milgamma, Vitrum antioxidant in complex therapy of the convalescent period of haemorrhagic fever with renal syndrome in 232 HFRS convalescents who were treated on an outpatient basis in polyclinics No 1 and No 2 of the Ufa City Clinical Hospital No 13. 113 convalescents after moderate HFRS (57 individuals along with standard therapy received an immunomodulator Imunofan and a vitamin mineral complex) and 119 convalescents of severe HFRS (64 individuals along with standard therapy received Imunofan and a vitamin-mineral complex). A healthy control group comprised 44 practically healthy individuals of the corresponding age. Concentrations of vitamins and cytokins were examined: interferon-γ (IFN-γ), tumour necrosis factor-α (TNF-α), interleukin-2 (IL-2), IL-4, IL-6, IL-10. Results. Immunological status was studied 1–1.5 month after discharge from hospital (when usually a sick-leave certificate expires and convalescents begin to work) and showed a reliable presence of immune deficiency and cytokine imbalance. Examination of vitamin status in HFRS convalescents immediately after discharge from hospital showed presence of А, Е, В1 В6 and С hypovitaminosis in both the winter and summer-autumn periods, which was more expressed aftera severe course of disease. As was found, this therapy contributes to a statistically significant decrease of TNF-α, IL-4, IL-10 levels and an increase of IFN-γ and IL-2 levels, has a normalising effect on parameters of cell-mediated immunity, enhances the phagocyte functional reserve capacity. Normalisation of vitamin status in moderate HFRS and significant improvement of vitamin parameters in severe HFRS have been pointed out. Conclusion. A positive effect on immunity, vitamin status, levels of essential trace elements promotes a clinical improvement of disease.
